Please schedule your lab draw the weekend before your upcoming doctor appointment. If your doctor's appointment is on Monday or Tuesday, please schedule your blood draw 2 weekends in advance.

Fairfax, Prince William, Frederick and Fauquier, Clarke, Jefferson County.

After booking your appointment online, under our blood collection (routine, specialty kits, and self-pay)
Please fill our registration form along with our self-pay requisition form. These two forms MUST be filled out and will be required to complete your booking.
Once completed, you may email these two forms to cpt@xpsnova.com or by fax to 703-327-8647. Failure to provide these forms will result in your appointment being canceled. If you are unable to send these forms back, please contact XPS LLC at 571-206-2848.

After you completed your appointment booking and filled out the registration form along with the self-pay requisition form, please use our Self-pay online secured portal to pay for your lab test.
Your lab test MUST be paid prior to lab send out. Failure to make payment will result in higher lab test cost which will come from the processing laboratory. Once labs have been sent out, WE CAN NOT change billing.

ALL services will be conducted during the weekend during our temporary weekday closure. Umbrella care discounted prices will be honored. If you need a lab draw during the week, please contact XPS LLC at 571-206-2848, non-umbrella care prices will be applied.
